Lithuania Schengen Visa Application Requirements

Lithuania Visa Application Requirements

1. Application form, completed truthfully in print and signed by the applicant in both required boxes. (Applications of minors have to be signed by their legal guardian). You can see the Schengen Visa Application Form here.

2. One (1) recent passport photo 35×45 colorful, with white background.

3. A valid travel document – passport (containing at least 2 blank pages, issued within the previous 10 years, its period of validity extending at least three months after the intended date of departure from the territory if the Member States).

4. Valid travel health insurance, covering the duration of stay and its copy. The amount of medical insurance coverage – EUR 30 000.

5. A confirmed return air-ticket reservation.

6. An approved letter of Invitation by the Migration Department of Lithuania for the alien to temporarily stay in the Republic of Lithuania or the documents confirming the purpose and conditions of the planned visit to the Republic of Lithuania (hotel booking, tour itinerary etc.).

7. The available means of subsistence during the period of his stay in the Republic of Lithuania or their lawful origin, the available means to return to a state which the alien has the right to be admitted to ( last 3 months original bank account statements/pay slips etc.).

8. Additional documents proving the purpose and conditions of the travel.

9. To obtain a multiple-entry short-stay visa, a foreign national must submit documents substantiating of the necessity for such visa to Visa Section or documents substantiating the circumstances entitling him to a multiple-entry short-stay visa.

10. Consular fee- 60EUR (cash).

Latvia Schengen Visa Application Requirements

Latvia Visa Application Requirements

1. Visa application form completed online, printed and signed by the applicant. You can see the Schengen Visa Application Form here.

2. Two (2) photos (size 35mm x 45 mm) must be taken in last 6 month and it should be on white background.

3. Passport with a period of validity extending at least three month after the intended date of departure from Latvia or other Member State, containing at least two blank pages, and issued within the previous ten years. Copies of the identification pages and the pages that contain information about previously issued visas.

4. Travel medical insurance valid for the whole Schengen area and covering the entire period of the intended visit in the Schengen area and additional 15 days – “the period of grace”. The minimum coverage of 30,000 EUR including the costs of urgent medical attention and/or emergency hospital treatment in case of an illness as well as repatriation in the event of death. Medical insurance policy can be obtained in applicant’s country of residence, or in any other country, or taken out by the inviting person. It is important that the insurance meets all the above requirements.

5. Invitation approved by the Office of Citizenship and Migration Affairs. The invitation should be dated not later than 6 months prior to the application date.

6. Round trip tickets: The flight tickets must be bought and confirmed by the airway company.

7. Proof of accommodation:
a. Hotel booking and proof of sufficient means to pay for the accommodation.
b. Hotel booking is not required if the accommodation is included in the Invitation.

8. Proof of sufficient financial means:
a. Original private bank account statement showing the movements during the last three months, duly stamped and signed by the bank;
b. Income tax return acknowledgment for the last two assessment years.

If the applicant is employed
a. pay slips for the last three months;
b. employment contract;
c. employer’s statement on approval for holidays;

If the applicant is a company owner or self-employed
a. certificate of registration of the company;

If the applicant is sponsored
a. proof of sponsorship and/or information on the Invitation;

If the applicant is retired
a. pension statements for the last three months;
b. proof of regular income generated by ownership of property or business.

9. If the applicant is a minor
a. If the minor travels with one parent only, written consent certified by public notary of the other parent or guardian, except in cases of a parent having the care or custody alone;
b. If the minor travels without parents, written consent certified by public notary of both parents or guardians having the care and custody of the applicant;
c. Copies of the passport of the parents. If not applicable, birth certificate of the applicant and copies of the ID cards of the parents.

10. Journeys undertaken for the purpose of tourism
a. Certificate of the travel agency confirming the booking of an organized trip or any other appropriate document indicating the envisaged travel plans;
b. Marriage certificate, if applicant is married and cannot provide evidence of marital status in the passport; copy of the ration card, if applicable.

Thailand Tourist Visa Application Requirements

Thailand Tourist Visa Application Requirements

1. Passport or travel document with validity not less than 4 months with two blank pages. Passport copy of front and last page

2. Visa application form completely filled in and signed by applicant.

3. Two color photographs of the applicant as per the specification*

4. Visa fees

5. Evidence of Transportation – confirmed flight itinerary or flight tickets in and out of Thailand

6. Proof of confirmed hotel booking or accommodation reservation in Thailand with address and contact number under applicant’s name. If travel in group, must provide a cover letter stating all names with passport numbers, plan of travel, and place(s) to stay.

If intend to stay with relatives or friends, must provide invitation letter along with copy of their ID/ passport and in case they are not Thai, must provide their work permit, valid at least six months.

7. Evidence of adequate finance (must cover both traveling and lodging expenses)

a. Bank statement from last month or
b. Copy of credit card with applicant’s name visibly seen and credit card statement from last month or
c. Endorsement of $700 per person under applicant’s name and in original

8. International Health Certificate on Yellow Fever Vaccination for applicants traveling from 45 countries

New Zealand Tourist Visa Application Requirements

New Zealand Tourist Visa Application Requirements

1. Application Form
A properly completed and signed application form. Please use the appropriate form:

a. A Visitor’s Visa Application form (INZ 1017) or
b. A Tourist/Business Visitor Visa Application form (INZ1189). Use this form if you intend to visit New Zealand for less than 6 months for tourism (holiday or visiting family/friends) or business purpose ONLY. You must be able to provide your own funds or your employer is sponsoring your visit.

2. Two (2) recent passport sized photos

3. Fees

a.  All fees are payable in Singapore Dollars and they are NONREFUNDABLE.
b. Visa fee – refer to the Visa fee finder.
c. VAC service fee and return courier fee.

4. If you are paying by bank draft, please make it payable to VFS Singapore Pte Ltd

4. VAC CREDIT CARD AUTHORISATION FORM FOR MAIL APPLICATION OR CREDIT CARD HOLDER NOT PRESENT AT VAC COUNTER. If you are paying by Visa or Mastercard credit card, please complete the VAC Credit Card Authorisation form for payment of visa fee, VAC service fee and return courier fee.
Note: Payment of courier fee is not required if passport is self-collected at the VAC

5. Original current passport or certified copy of the passport bio-data page

6. For non-citizens, evidence of your residence status document such as IC, employment pass, dependant pass, long-term visit pass or  student pass

7. For non-citizens, evidence of travel to other countries, eg. copy of visas, entry stamps

8. IF SECONDARY APPLICANTS ARE INCLUDED IN THE APPLICATION FORM, please submit evidence of your relationship to them, such as your marriage certificate, your child’s birth certificate showing parents’ names.

9. ENGAGED COUPLE: Submit separate visa applications unless you can provide evidence of your partnership and evidence of living together in a genuine and stable relationship.

10. LETTER OF CONSENT: If your dependent child is included in your application but the child’s other parent is not travelling with you, please provide a letter of consent for the
child to travel, from the non-travelling parent

11. A copy of your travel booking.
Note: Please do not purchase your flight ticket until a decision is made on your visa application. You can submit a flight reservation.

12. Evidence of funds
Funds of NZ$1,000 per person per month or NZ$400 per person per month and evidence of prepaid accommodation. You can provide:

a. Letter from your bank or bank statements for the last 3 months in your name if you are self-sponsored, OR
b. A properly completed and signed ORIGINAL Sponsorship Form for Temporary Entry (INZ 1025) completed by your New Zealand sponsor (please submit supporting documents as specified in the form)

13. HEALTH REQUIREMENTS

a. If your stay is more than 6 months but less than 12 months – submit a Chest X-ray Certificate (INZ 1096).
b. If your stay is more than 12 months – submit a General Medical Certificate (INZ 1007) & a Chest X-ray Certificate (INZ 1096)
c. The medical certificates must be less than 3 months old at the time your application is submitted and completed by an INZ panel doctor. Refer to our Panel Doctor list.
DO NOT SUBMIT the chest x-ray film or CD if your x-ray result is normal.
Children under 11 years of age and pregnant women are not required to submit a chest x-ray.

Please note:

Your General Medical & Chest X-ray certificates are valid for 36 months (based on the certificate’s date of issue) when they are accepted by INZ. You need not submit a new one unless you have been advised otherwise.

If you have spent the last 6 consecutive months in a country that is not a low-incidence country for tuberculosis since departing New Zealand and you intend to stay in New Zealand for more than 6 months, you must submit a NEW Chest Xray certificate.

14. Please provide any one of the following:

a. If you are employed, a letter from your employer stating the position you hold, your length of employment, monthly salary and leave approval dates granted.

b. If you are a student, a letter from your school stating the course you are attending, start and end date of your course and leave approval dates granted.

c. If you are a student traveling on a school organised trip, a letter from your school confirming the trip and payment of trip, a letter of consent (if you are under 16 years old) and financial support from your parent or guardian and evidence of their relationship to you.

d. If you are self-employed, latest income tax statement and company registration (eg. ACRA business profile, company registration).

Brazil Tourist Visa Application Requirements

Brazil Tourist Visa Application Requirements

1.  Passport valid for no less than six (6) months with two (2) blank pages. Spoiled, damaged or defaced passports will not be accepted;

2.  Properly filled on-line visa application form. Fill it out, print the receipt and sign;

3. One (1) recent colored photo (passport-size) full-face, white background, duly pasted on the on-line visa form. Photos must be taken not more than six (6) months prior to submission;

4. Complete itinerary with passenger’s name. The itinerary should mention when and where the passenger is entering and leaving Brazil;

5.  Evidence of sufficient funds for self-maintenance during the stay in Brazil;

6.  Other document includes marriage certificate if travelling with spouse, birth certificate for minors.

7. Payment of Consular Fee;

8. Letter of authorization from the interested party or company will be requested if the passport is to be delivered to a third party;

9. Additional documents may be requested at the Consular Authority’s discretion.

South Africa Tourist Visa Application Requirements

South Africa Tourist Visa Application Requirements

1. Visa application form. Completed Visa application Form (DHA-84) Form 11 in BLACK ink only.

2. Two (2) recent colored photograph. Must be a clear copy, not older than 6 months, size 3.5 cm × 4.5 cm, white background.

3. No-objection letter from employer or sponsor (Original)

a. It must be signed and stamped; dated/issued on the current month of should be dated not more than a month from the date of application.
b. The letter must confirm the travel dates, exact purpose of travel and “no objection” from the sponsor/employer for the said travel. Full contact details of the company/employer (e.g. physical address, telephone number) must be indicated in the letter.
c. For self-sponsored, valid trade license copy. Applicants under the sponsorship of a husband/father/mother/wife/employer visa
d. Typed and signed Sponsorship letter (issued by the sponsor, it must confirm the travel dates, the exact purpose of travel, relationship, details of the applicant, and financial undertaking). The financial undertaking from the sponsor is required, only if, the applicant itself could not provide his/her personal bank statement (e.g. minor, unemployed, student, housemaid, etc.). Sponsor’s residential address in the UAE and contacts details must be indicated in the letter.
e. Passport and UAE visa copy of the sponsor
f. If the applicant could not provide his/her own bank statement, the 3 months personal bank statement of the sponsor is required (must be stamped by the bank).

For applicants MARRIED to a SOUTH AFRICAN, the following additional documents are required:
a. Copy of Marriage certificate
b. Copy of South African ID or South African passport of the spouse

4. Ticket. Proof of flight itinerary or airline ticket (provisional or confirmed). Proof of Domestic flight/s is also required, if the applicant is travelling from one city to another

5. Accommodation
If staying in a hotel, a proof of confirmed hotel booking or provisional hotel booking is required
If staying with a family/relative/friend/colleague, a copy of the following from the SA host are required.

a. Invitation letter (typed, signed and must be dated/issued on the current month or should be dated not more than a month from the date of application). The physical home address and contact details of the SA host should be indicated in the letter.
b. SA host copy of his/her latest three (3) months personal bank statement
c. Proof of address (e.g. Utility bills, Lease of Contract, Tax invoice, etc.) as indicated in the invitation letter.
d. If the host is a South African, a copy of South African ID book or passport is required. If the host is not a South African, a copy of passport and residence/permit/visa copy of the host in South Africa is required.

For parents and siblings visiting either their son/daughter/siblings in South Africa, and will be staying with them whilst in South Africa, the following additional documents are required:

a. Copy of son/daughter/sibling’s full birth certificate (In English only)
b. Copy of son/daughter/sibling’s passport
c. Copy of visa/temporary residence/permanent residence permit issued/granted to them

6. Medical Cover. Submit a valid travel insurance must be submitted.

7. Proof of financial means.

a. Applicant’s latest three (3) months banks statement. ONLY bank statements issued by a bank “in the UAE” are acceptable.
b. original copies received by post OR
c. print out copies from the online internet banking (but it must be stamped by the bank)
d. Should the applicant submit the bank statement of the company, the following additional documents are required:

-Letter from the company/sponsor which states that the company has no objection to use the bank account/statement by the applicant, to cover his/her travel expenses in South Africa. The letter must be signed by ALL partners/owners of the company (as shown in the trade
license).
-Copy of Trade License.
-Copy of ALL partners/owners passport and UAE visa page.
-3 months latest bank statement of the company (stamped by the bank)

8. Yellow fever certificate
A yellow fever certificate is required “ONLY IF” an applicant is coming from or going through a yellow fever endemic zone. Applicants who are going straight/directly to South Africa from Dubai, and who have not been in a yellow fever zone recently or within a year are “NOT”

9. Passport
Must be valid for no less than 30 days after the expiry of intended visit to South Africa and containing at least 2 blank visa pages (one to endorse/stamp the visa and the other one for entry/exit stamps. Residence permit must be valid for at least 3 months from the date of
application.

Validity should not be less than 3 months from the date of application. Issuance date must be 3 months and longer on the time of application. Photocopies of data page, residence permit and previous South African Visa/s (if any) must be submitted.

Turkish Tourist Visa Application Requirements

Turkey Visa Application Requirements

1. Visa Application Form duly filled, submitted on the online system, printed and signed by the applicant. (CNIC number, father and mother names, complete residential and host (if applicable) addresses are must.)

2. Recent and old Passport Copies along with all used (VISA & entry-exit stamp) pages copies. Passport or other recognised travel document should be valid for at least 180 days. Other Third Country Nationals who resides in Pakistan must provide their valid visa or residence permit in Pakistan.

3. Two (2) recent biometric photographs (white background) of the applicant on a white background with 5cm x 5cm size.

3. Employment Cover Letter of the applicant’s company/employer (where applicable) stating name, position, salary, duration of employment along with last three months’ salary slips. Government employees must provide the Ex-Pakistan leave or NOC from relevant departments for the visa application along with last three months’ salary slips

4. Valid Police Character Certificate. In accordance with Foreigners and International Protection Act no. 6458, article 15/c, all
foreigners wishing to apply for Turkish visa shall submit a Valid Police Character Certificate.

5. Travel Health Insurance Certificate. In accordance with Foreigners and International Protection Act no. 6458, article 15/e, all foreigners wishing to apply for Turkish visa shall submit a Travel Health Insurance Certificate covering the duration of their stay in Turkey and as well as repatriation costs.

6. Copy of flight and Hotel booking. This is your flight reservation or flight ticket as well as your Hotel or Accommodation Reservation

7. Bank Account Maintenance Certificate and Original Bank Statements of the last three months’ which are signed and stamped by the bank officer.

Conditional Documents:

1. Invitation Letter (if any) with identity proof such as ID Card / residence card of invitee.

2. Invitation Letter (only for Afghan Nationals) which is attested by a Notary Public.

3. Chamber of Commerce Membership Certificate and it’s copy (If applicable)

4. National Tax Number (NTN) Certificate and its copy. Proof of income tax payment with FBR (where applicable)

5. E-Visa copy/copies. If accompanied by any person who has a valid Turkish E-Visa

6. Consent Letter for Underage Applicants (under 18) travelling with a single parent or alone, a written consent of the other parent /
both parents is compulsory with photocopy of his/her CNIC on a stamp paper. Students under 18 years old travelling with school groups, etc. must provide consent from both parents with copies of their CNIC on a stamp paper.

7. If the applicant is a student, a Bonafide Letter signed by the applicant’s school/college or University. Affidavit Letters from parents or their sponsors of the students regarding their expenses along with their bank statement.

8. Original Marriage Certificate (MRC) (husband/wife/children) travelling together. Applicants travelling without any family members (solo) do not need to provide MRC. If the husband name is mentioned on the Passport then MRC is not mandatory.

9. Professional Skill Certificates like Pakistan Medical & Dental Council (PMDC) Certificate Copy for Doctors, Bar Letter for Lawyers, Pakistan Engineering Council (PEC) Certificate Copy for Engineers, Valid Tourism Licence Copy for the Travel Agency owners and their employers.

Chinese Tourist Visa Application Requirements

China Visa Application Requirements

1. Visa Application Form – You must submit truthfully completed and signed Visa Application Form of the People’s Republic of China. Do not leave any field blank. Write N/A if the question does not apply to you.

2. Passport – Original passport that is valid for at least another 6 months with at least one blank visa page, a photocopy of the passport’s information/photo page and emergency contact page. All passports issued on 2014 onwards must attach the original OLD passport upon filing (with or without previous China visa).

3.  Two (2) pcs recent colored photo – in white background, 48mm x 33mm size, front view, without head covering.
a. Affix (glue) one photo on the appropriate field of the application form. Stapled/ taped/ clipped/ detached photos will not be accepted.
b. Insert another photo in a small clear/transparent plastic, stapled at the upper right corner of the first page of application form.

Photo size should be 33mm (width) x 48mm (height). The head width should be between 15mm and 22mm and the head height,
measured from the base of the chin to the crown of the head, should be between 28mm and 33mm. The space between the
crown and the upper edge of the photo should be between 3mm and 5mm. The space between the chin and the bottom edge of
the photo should be ≥7mm.
The photo should be clear, no damage or impurities, no background light or shadow over face.
Background of the photo should be white (gray / dirty white photo background will not be accepted)
Wearing white shirt is strictly not allowed.
Texture of photo must be glossy (matte-type of photo paper is not allowed)

4. Travel itinerary and reservation of round-trip flight ticket and hotel booking, or invitation letters from China.

Additional Requirements:

5.  First Time Chinese Visa Applicant

Bank certificate and the receipt for payment of this certificate. Bank Certificate should indicate the following information:
a. Account opening date
b. Type of account (SAVINGS or CURRENT)
CURRENT ACCOUNT: Original copy of Statement of Accounts – transaction for 6 months
SAVINGS ACCOUNT: Original UPDATED PASSBOOK
c. Average Daily Balance (ADB)
To date or current balance
With check write
d. The types of Bank Certificate acceptable are savings or current savings from commercial bank only.
e. Individual Bank Certificate should be open at least 6 months prior to applying for visa.
f. Individual Bank Account should have an AVERAGE DAILY BALANCE AND TO DATE CURRENT BALANCE not below PHP 100,000
per applicant.
*TAKE NOTE that the PASSBOOK BALANCE should match the BANK CERTIFICATE CURRENT BALANCE.
**If there is AND/OR in ACCOUNT NAME in passbook, please provide proof of relationship such as NSO BIRTH CERTIFICATE and/or
NSO MARRIAGE CERTIFICATE.
BIR-stamped income tax return form,
Letter from your employer detailing your salary and the length of your employment,
Business registration certificate if you are the owner, or professional ID, or student ID, etc.
*A letter of explanation is required if you could not provide the above-mentioned documents.

6. Previous Chinese Visa – If you had obtained a Chinese visa before, you should submit a photocopy of the visa, and if the visa is on your old passport, you should also submit the old passport.

7. Non-Filipino Nationals – You must submit proof of residency permit, employment or study in the Philippines. Temporary visitors to the Philippines should apply for Chinese visas from the Chinese Embassy/Consulate in the countries in which they live, unless the applicant can provide proof of employment/study or other occupational status and the visa officer will make the final decision on a case-by-case basis. You must also submit a photocopy of the Philippine visa/immigration entry stamp on your passport.

8. Certificate of Name Change – If the name in the new passport is different from that in the original one, the official document (birth certificate/marriage contract) issued by authorities for name change must be submitted.

9. Original Chinese Passport – First-time applicant whose former nationality was Chinese, or who was born in China (including Hong Kong, Macao and Taiwan), need to submit the original Chinese passport and a photocopy of the passport’s information/photo page, and extension page (if applicable). You do not need to submit income tax return form and bank statement.

10. Visit Tibet – You may join a tourist group organized by Authorized Travel Agency.

11. Applicants applying for a special tour to China (hiking, cycling, horse-riding, hot air balloon, or other expeditions) should provide an original of Invitation Letter for Tourist issued by Authorized Travel Agency. Authorized Travel Agency refers to:
a) Tourism administration bureau of Chinese province, autonomous region or municipality directly under the Central Government.
b) CITS Group Corporation, CITS Head Office, CTS Head Office, CYTS, etc.

13. Seafarers from the Philippine shipping companies to join vessels in ports in China and stay in China for less than 10 days, should submit Letter of Guarantee from employers, and the original and copy of Seafarer’s Registration Certificate, and should apply for “G” visa. Those who would stay in China for more than 10 days should also submit invitation letters from China, and should apply for “M” visa.

14.  American passport holders may apply 10 years multiple entries L/M/Q2/S2 Visa, or 5 years multiple entries X1 Visa, only if his/her passport is valid over 1 year from the date of application.

Japan Tourist Visa Application Requirements

Japan Visa Application Requirements

1. Visa Application Form. Application form should be filled out all items correctly, If item is not applicable, please fill in
[N/A]. Embassy may not accept application with blank item, no signature or no date in the form; may deny the application that is filled out incorrectly or wrong information.

2. Valid Passport. Broken passport is not accepted. Passports must be signed and must have at least two (2) blank visa pages.

3. Photo. Specs: 4.5cm x 4.5cm, with white background. Photo must be taken within 6 months. Please write applicant’s name and birthdate on back side of the photo. Photo must be pasted on the application form.

4. Birth Certificate. Must be issued within one year from PSA Main Office/Serbilis Outlet Center (Nationwide).

a. If the birth certificate from Statistics Office is unreadable, or has incomplete information, please submit the birth certificate issued by PSA together with a birth certificate issued by the Local Civil Registrar.
b. If the birth certificate is “late registration”, please submit in addition, a baptismal certificate and school record (Form 137) from high school or elementary and School Yearbook (if possible).
c. If there is no record in the statistics office, please submit a “Certificate of Non-Record” together with a birth certificate from the Local Civil Registrar.

5. Marriage Certificate (if the applicant is married). It must be issued within one year. In case there is no record, please submit a “Certificate of Non-Record” from the statistics office together with the Marriage Certificate from the Local Civil Registrar

6. Daily Schedule in Japan

12. Foreign residents with long-term or permanent status in your country applying for a visa must submit, in addition to the above requirements (except birth and marriage certificates), a copy of their Alien Certificate of Registration (ACR) issued by the government.

Visa application of foreigners who are on short-term (temporary) visa in your country cannot be accepted. Please apply at the Japanese Embassy /Consulate General with jurisdiction over the area in which the applicant resides or over the country of which the applicant’s
passport was issued.

Size of document for application should be A4 size only. If document is other size, please submit copy that is already adjusted in A4 size from its original and without staples or pasting pages.

Applicants are exempted from submitting (4) and (5) Indicated above if they can submit old/valid passports with used Japan visa.

It is applicant’s responsibility to ensure that he/she meet the requirements for the grant of a visa. Submission of any supporting documents not listed above is encouraged (e.g. applicant’s economic or social ties with his or her country, urgent reasons for visit: medical certificate, wedding invitation).

United States of America Visa Application Requirements

1. A Nonimmigrant Visa Electronic Application (DS-160) Form.

2. A passport valid for travel to the United States with a validity date at least six months beyond your intended period of stay in the United States (unless country-specific agreements provide exemptions). If more than one person is included in your passport, each person desiring a visa must submit an application.

3. One (1) 2″x2″ (5cmx5cm) photograph

4. A receipt showing payment of your US$160 non-refundable nonimmigrant visa application processing fee, paid in local currency. If a visa is issued, there may be an additional visa issuance reciprocity fee, depending on your nationality. The Department of State’s website can help you find out if you must pay a visa issuance reciprocity fee and what the fee amount is.

5. Current proof of income, tax payments, property or business ownership, or assets.

6. Your travel itinerary and/or other explanation about your planned trip.

7. A letter from you employer detailing your position, salary, how long you have been employed, any authorized vacation, and the business purpose, if any, of your U.S. trip.

8. Criminal/court records pertaining to any arrest or conviction anywhere, even if you completed your sentence or were later pardoned.

9. For students: latest school results, transcripts and degrees/diplomas. Also bring evidence of financial support such as monthly bank statements, fixed deposit slips, or other evidence.

Working adults
Bring an employment letter from your employer and pay slips from the most recent three months.

Business visitors and company directors
Bring evidence of your position in the company and remuneration.

Visiting a relative
Bring photocopies of your relative’s proof of status (e.g. Green Card, naturalization certificate, valid visa, etc).

Previous visitors to the U.S.
If you were previously in the United States, any documents attesting to your immigration or visa status.
